Vision add-on

A bundled vision benefit that rides alongside some dental PPO plans.

What is a vision add-on?

A dental vision add-on (also called a dual-benefit or bundled plan) combines basic vision insurance benefits with a PPO dental plan in a single monthly premium. Vision coverage typically includes one comprehensive eye exam per calendar year and an annual allowance toward prescription eyeglass frames/lenses or contact lenses. These bundles are most commonly offered by carriers like Humana (Extend series) and select employer benefits packages. The dental and vision components have completely separate benefit structures, deductibles, and maximum amounts, being in-network for dental does not mean the same provider network for vision.

How it works

The dental component functions exactly like a standalone PPO: deductible, coinsurance, annual maximum, waiting periods. The vision component follows vision-specific rules: the annual eye exam is covered at 100% or low copay; frames/lenses have a fixed allowance (typically $100 to $200) with the member paying the overage. Some bundles include contact lenses in lieu of glasses. Claims are adjudicated separately by the dental and vision divisions of the same carrier.

Vision: comprehensive exam covered at $0 copay in-network. You choose frames costing $280, you pay $280 − $150 allowance = $130 out-of-pocket for frames. Lenses: covered at $0 for basic lenses. Total vision out-of-pocket: $130.

What to watch out for

  • The vision network is separate from the dental network. You cannot see your dentist's affiliated optometrist and assume they're in the vision network, verify separately using the carrier's vision directory (often LensCrafters, Pearle Vision, or VSP-affiliated practices for bundled plans).
  • Bundled plans rarely offer the richest dental benefits. The best standalone dental PPO plans typically have higher annual maximums and broader coverage than bundled products. Compare the total dental benefit against standalone dental + standalone vision costs before choosing a bundle.
