        The plain-English shelf

        The terms that decide your bill.

        Annual maximum
        The most a dental plan will pay toward your care in one year. Once you reach it, you cover the rest yourself.
        Waiting period
        The number of months you must hold a plan before a tier of dental care becomes covered.
        Deductible
        The amount you pay out of pocket each year before your plan begins sharing the cost of care.
        Coinsurance
        The share of a covered procedure you pay after the deductible, such as 50% of the cost of a crown.
        In-network
        A dentist who has agreed to a plan's discounted PPO fees, so your share of the bill is lower.
        Major services
        Higher-cost dental work such as crowns, bridges, dentures and implants, usually the last tier covered.
        Missing-tooth clause
        A rule that excludes coverage for replacing teeth you lost before the plan started.
        Out-of-pocket cost
        The portion of a dental bill you pay yourself after the plan has paid its share.
        Balance billing
        When a dentist bills you for the difference between their fee and the plan's allowed amount.
