12-month major wait

Crowns, root canals, implants and orthodontics each wait a full year. Proof of prior continuous coverage can waive it in many states, but plan around it if you need work soon.

Missing-tooth clause

A missing-tooth exclusion applies to PPO Premium renewals from August 2025, except in California. A tooth lost before your effective date may not qualify for an implant or bridge.

Whitening is state-specific

Teeth whitening is not offered in every state. Where included on Premium, it is commonly 25 to 50% after a 6-month wait, ages 16 and up. Do not assume it applies in your state.
