What is preventive care?
Preventive dental care includes routine cleanings (prophylaxis), comprehensive and periodic examinations, bitewing and full-mouth X-rays (within frequency limits), fluoride treatments, sealants for children, and oral cancer screenings. PPO plans cover preventive services at 100% of the in-network contracted fee from the policy's first day, with no waiting period and no deductible. These services exist to prevent more expensive restorative problems — insurers cover them fully because a $120 cleaning is dramatically cheaper than a $1,200 crown that a missed cavity would have eventually required.
How it works
Preventive claims submit under CDT codes beginning D0 (diagnostic, like exams and X-rays) and D1 (preventive, like cleanings and fluoride). The carrier verifies: (1) the code is preventive category, (2) the frequency limit has not been exceeded, and (3) the in-network provider's contracted rate applies. If all three check out, the plan pays 100% and the member pays $0 in-network.
You visit your in-network dentist twice a year. Each visit includes a cleaning (D1110) and periodic exam (D0120). January visit: dentist also takes bitewing X-rays (D0274, 4 images). All covered 100%. Patient cost: $0. July visit: cleaning and exam. No X-rays (frequency limit: once per calendar year for bitewings). Patient cost: $0. Total preventive cost for the year: $0.
What to watch out for
- Frequency limits are strict. Two cleanings per calendar year is the standard. A third cleaning — even if your dentist recommends it — is typically denied and becomes 100% out-of-pocket unless your plan has a periodontal maintenance benefit (D4910) for diagnosed gum disease.
- Sealants are typically covered only for children under a specific age (usually 14 or 16) and only on permanent molars. Adult sealants are often excluded or covered at a reduced rate.

Most PPO plans cover two cleanings per calendar year — typically one every 6 months. Some plans specify the frequency differently (once per 6 consecutive months) rather than twice per calendar year, which affects timing for patients who want both visits in the second half of the year.
Yes. Bitewing X-rays (D0274) are covered preventively, typically once per calendar year. Panoramic X-rays (D0330) are covered less frequently — usually once every 3–5 years. If your dentist diagnoses periodontal disease and prescribes more frequent cleanings as medical necessity, additional visits may be submitted under D4910 (periodontal maintenance) instead of D1110 (prophylaxis).
